It depicts American personification Brother Jonathan refusing a bleeding treatment by a doctor, referring to George Washington (depicted on the right). The caption reads:

Doctor, (the Enthusiastic.) "My dear friend, your pulse is very full–you really don't know your own constitution–you positively must lose a little blood."

Jonathan, (the Patient.) "It's all very well, Mister, in your opinion, but my Old Doctor thinks otherwise"
