A formal disambiguation is a disambiguated title that is created by adding a disambiguator to a base name, that is, any disambiguated title other than a natural disambiguation. In Wikipedia, comma disambiguation and parenthetical disambiguation are both examples of formal disambiguation.
Or to put it another way, formal disambiguation is the sort that works with the pipe trick, while natural disambiguation doesn't (and can't) do this.
