The fact that one character in one episode wears the same t-shirt as another character in another episode three years later is not continuity. There is nothing continuous between these two cases. You are confusing continuity with trivia. The "definition of continuity" would be elements of the plot of a particular episode that depend on knowing something from a previous episode to understand. Some continuous story shows have a "previously on" segment at the beginning to provide this continuity. Elements of an episode other than this can be continuity items if there is some significant connection between two episodes. But in this case there is no reason to think that two different characters happening to wear the same shirt several years apart from each other are connected at all.
