The majority of the necessary citations can be found in History of the First Unitarian Society of Chicago, 1836-1936 (Chicago: First Unitarian Society, 1936) by Esther Hornor. However, the book is only available in print copies as near as I can tell. I leave it to a more interested and talented wikipedian to solve this dilemma.
