In 1920 Who's Who (cited here) he is Vaughan, Herbert Millingchamp, J.P., F.S.A.
In one 1916 newspaper listing Meleager he is credited as H. M. Vaughan, F.S.A. ("Books Received by The Sphere", The Sphere 1916-01-29 p106).
In library records presumably from title pages he is Herbert M. Vaughan or H. M. Vaughan.
Not sure how to handle this I revised the second paragraph to begin boldface "H. M. Vaughan". --P64 (talk) 12:37, 22 August 2022 (UTC)Reply
