Chapter fifty's resume is wrong. The actual resume is from chapter thirteen from Black Jack 21, where Black Jack worries about Pinoko's safety and makes a deal with her: he saves the twins, but she must go back to Japan with Sharaku (who was there visiting his father). At the end, Black Jack notice her sadness and gives an excuse saying that there were no more plane tickets, so she must keep traveling with him.
About chapter fifty: two brothers (not twins!) lose their father to cancer. Both of them promise him to become doctors, but the older brother decides to take care of family bussines and give his younger brother a chance to study. The younger brother, due to the pressure of it, fails at the exams and ends up running away from home. Later, he funds a succesful company and wants to associate with his brother, but the older denies (feeling betrayed by his younger). Soon after that, the older brother fells ill. So the younger hires Black Jack (who acepts the job thanks to Pinoko pleas for new pots) to act as his son, so the older would agree to be operated by his "recently graduated as a doctor nephew" of his cancer. Altough the ill brother notice the cheat, Black Jack brought the real nephew to operate in secret, so after some emotive apologizes the brothers are reunited again.
