Edit Suggestions UCSF Wikiproject Medicine 2017

Hello! I'm a medical student with UC San Francisco Wikiproject Medicine and editing this page over the course of the next month. One of my peers will aid me in reviewing my page as well. I have compiled a list of suggestions to improve this article and I would appreciate community feedback. I have updated the list below as my work plan has evolved.

  1. Use Acrolinx throughout the document to improve overall readability
  2. Expand the lead to reflect each section of the article
  3. Expand the transmission section to include perinatal transmission and genito oral transmission.
  4. Reorganize the Disease section. Specify male GU infection/female GU infection/neonatal infection/anal infection/pharyngeal infection/disseminated infection. Will see what CDC says about rates of asymptomatic infection and rates of infertility.
  5. Reorganize treatment and prevention, move the sentence about "patients should also be tested for..." to Diagnosis. And make co-treatment for chlamydia its own paragraph. Change latex barriers to "physical barriers" as both condoms and dental dams are not necessarily latex.
  6. Update Treatment and Prevention to include the latest treatment guidelines from the CDC. Ceftriaxone is no longer reserved until after penicillin resistance has been proven.
  7. Under Treatment and Prevention Add a subheader for antibiotic resistance, for prevention.
  8. Add subheaders under microbiology for morphology, culture.
  9. Move the "survival of gonorrhea" as a subheader under microbiology rather than its own section
  10. Update diagnosis. Culture is not always required for gonorrhea diagnosis, but rather depends on the type of infection (male vs female, pharyngeal, anal), we mostly use nucleic acid amplification now for screening and diagnosis of genital infection.
  11. Add a history section to describe the origin of the name and the series of discoveries that let to the discovery of the organism (not the whole history of gonorrhea the infection).

PEER REVIEW of above student's work
Lead: Lead length is great. I know guidelines say 1 paragraph per section, but I would consider omitting the last paragraph in the lead pertaining to history. Seems unimportant, and is kind of an abrupt change from an otherwise cohesive lead. 
References: Add references for facts in the lead section (sexual transmission paragraph + treatment paragraph). Seems like you added a citation at the end of each of those paragraphs, but I believe you’re supposed to add a reference at end of each sentence, even if they are from the same source. 
Disease section: Disease section is perfect. Great job expanding this section! Really important that you explained all the other possible areas of infection, as most people just associate gonorrhea with the genitals. I also liked how you said “infection of the throat” instead of just pharyngitis. I might use that same style to describe meningitis and endocarditis, which appear at the end of this section
Diagnostic methods: You mention PCR based methods are the most commonly used, but there is not much information on what exactly they entail. I would expand that a bit. The phrase “the diagnosis is made” is potentially confusing. Consider rephrasing to “diagnosis of male genital infection is made by visualizing…” I didn’t change this directly b/c not sure if that is the wording from the source and you were trying to reword it to avoid plagiarism. Also, I would briefly explain what gram stain is, and what culture is.
Other: I think wiki-links should be added the first time a word appears in the article. For ex: septic arthritis is in the first paragraph in the lead, but isn’t linked until much later. "Tetracycline in uncomplicated gonococcal infections"

i know tetracyclins are used for chlamydial infections, but what is this bit I just read about "uncomplicated gonococcal infections all about." I read it in a book with no further explanation. Hi there, editing this page as part of my graduate course.

Some initial thoughts as there are many detailed improvements this page requires:

1) The lead/introduction is a bit choppy (seems piecemeal created). Definitely should at least mention antibiotic resistance as this is the main concern with Ng currently in the scientific world and does a poor job of an overall summary of the article

2) Missing a citation after the last sentence of first paragraph in the lead

3) In surface proteins LOS is mentioned, which is in fact not a protein, a better section should probably be put together titled “surface molecules.” Additionally, LOS is referred to as LPS in at least one instance

4) There is mention of antigenic variation but not phase variation, both different but important aspects of persistent Ng

5) There is nothing on the colonization of the epithelial lining before transcytosis occurs

6) There is no information about the serum resistance of Ng and its use of complement regulatory proteins to evade complement killing, another important aspect of its survival

7) Antibiotic resistance should be updated as there are further confirmed cases of resistance now to combination treatments

8) There are citations and information taken directly from primary literature, and while this is intuitive from a scientist’s prospective, Wikipedia does not wish for this type of sourced information to be included in its articles (8, 13, 14, 15, 16, 21, 22, 23, 32, 35 is a news article (total of 10 of 43)

9) “Dynamic polymeric protein filaments called type IV pili allow N. gonorrhoeae to adhere to and move along surfaces.” Sounds plagiarized (no offense to the writer of this portion), although cannot confirm as I do not have access to the book which is cited

10) Survival of gonococci is a very vague section heading with little information. Technically this could include immune evasion, colonization, etc.

11) There is one subheading, organization definitely needs to be improved

12) Disease section is all about symptoms not the establishment of infection

13) The coverage seems to be neutral fortunately with an emphasis on proven scientific fact

14) The images in the article are helpful but they are specific for identification and drift all the way down throughout the article. A depiction of the lifecycle of infection would be helpful.

15) Not many people are working within the talk page with one another to improve the article


Overall a lot of restructuring of this article is necessary. References certainly need to be fixed to reflect secondary literature. Additionally, three is quite a bit of information missing from this article that is critical about Neisseria gonorrhoeae such as phase variation, the specifics of how it infects/interacts with the host cells, and evasion of killing by the complement system to name a few.

Let me know what you think about these and other potential issues with the article as there are certainly many. I will hopefully be making meaningful contributions to the improvement of this article throughout the following months.

Hello all,

I have done a substantial amount of editing in an attempt to better this article. A summary of my edits can be found below. -A creation of an infection subheading and information pertaining to the establishment of infection (entire section) -Edits to the antigenic variation section for clarity, and removal of Opa protein changes for inclusion in phase variation section -A creation of a phase variation subheading and information on this process during infection. -Inclusion of Opa protein information previously in article under the antigenic variation portion -Treatment section was updated to reflect most recent guidelines as stated by the CDC (and WHO) -Added serum resistance section, set up so that it should easily be elaborated on below the first paragraph -Cleaned up the flow of the disease section so it didn’t read as individual lines of text and random information -Amended the “surface proteins” section title to “surface molecules” to cover the discussion of LOS in that section

For those looking to edit this article in the future, please do not simply add free standing sentences of information that seemingly belongs in a section. Please look through the text and find the place that is appropriate and alter the paragraphs to reflect your intended changes. This will allow for much smoother editing and allow the article to be improved as a whole instead of creating more piecemeal parts that will need edited together.
