WikiMedicine Elective Project

As part of a school course, I will be editing this article for the next several weeks. My timeline is as follows:

Week 1: Identify areas of improvement, create timeline, start looking for reliable reference sources

Week 2: Write paragraphs for both the "diagnosis" and "signs & symptoms" sections (both are currently empty)

Week 3: Improve upon the lead paragraph and "causes" section. Consider adding additional sections such as prognosis, epidemiology, ect.

          I am a fourth year medical student involved in an elective editing wikipedia. This is my peer review of this article. 

         Overall good job! Very concise and easy to read. Good addition of symptoms and diagnosis. Flows easy. Appropriate sources, good links.

         Lead has good info, would consider clarifying “in most cases there is no specific treatment” For example, you could say something like, in most cases the virus will clear on its own without any   
         treatment. However, a few viruses, HSV, etc, we can treat with antiviral medication. Otherwise people might think there is no treatment and they will not get better.

         Small point but in the lead would write out cerebral spinal fluid not just CSF. 

         Diagnosis 
         I would recommend clarifying why a lumbar puncture is sometimes done before. The population is a good addition also, but a general reason i.e. increased ICP would be helpful for clarity.

         Treatment 
         Could include need for hospitalization, individuals who need IVF, etc. I think this is a common area of confusion, like if someone should seek evaluation, go to the emergency room, etc. 

         Epidemiology

2020 WikiProject Medicine

Hello! I am a forth year medical student involved with WikiProject Medicine. This 4 week elective course is designed to help improve existing Wikipedia medicine pages. I am excited to be a part of this community!

My overall goal for the next 4 weeks is to generally expand the already wonderful material on this page. Viral meningitis is such an important topic in health care, and I feel that it can be a confusing condition for students and patients alike. With this page being rated as "Start", I hope to fill in any potential gaps in information, add more images and videos, and increase the proportion of references that are systematic reviews. I have a particular interest in the pediatric population, and will increase the amount of information related to pediatric and infantile meningitis. More specifically, my goals include:

- Update the epidemiology in the Introduction. The current statistics are from 1988 to 1999.

- In the Signs and Symptoms category, I hope to make it a bit more reader friendly for lay people. Right now, there is some medical jargon surrounding fever and neck stiffness. Additionally, I will add more information on which populations are most susceptible to viral meningitis (immunocompromised). I think it could also add value to describe Cushing's Triad as it relates to increased intracranial pressure. This is an important possible set of symptoms to be on the look out for.

- I feel that the Mechanism section would be a great area to add a video on the pathophysiology of viral meningitis.

- In the Diagnosis section, I plan to add: a picture/video of a lumbar puncture, more information regarding CSF lab values, and a paragraph that discusses differential diagnosis.

- Add information about possible hospital observation in the Treatment section.

- Update the Epidemiology section with more recent statistics.

Lead evaluation

Overall, I thought the Lead section was adequate at introducing the topic and not overly detailed. According to the Talk page, VPedigo planned to update the epidemiology statistics in the Lead section because the existing statistics were from 1988-1999. The author has successfully done so, and the statistics have been updated with several sources (i.e. one from 2012, one from 2017).

I thought there were a few ways in which the Lead section could be improved. For instance, the introductory sentence is somewhat confusing. While the introductory sentence states that viral meningitis is synonymous with aseptic meningitis, the "Aseptic meningitis" article states that it can be caused by "viruses, mycobacteria, spirochetes, fungi, medications...", which suggests that viral meningitis is not synonymous aseptic meningitis and instead falls under a larger umbrella term. In addition, I thought that a few symptoms and signs could be mentioned in the Lead section.

Signs and Symptoms evaluation

Overall, I thought that this section was very strong; it was detailed, organized, and accessible to readers. VPedigo planned to make this section easier to read and add information about Cushing’s Triad, both of which were done. I particularly like that mechanisms for certain signs and symptoms were explained.

The only criticism that I have for this section is that the sentence “In severe cases, people may experience concomitant encephalitis (meningoencephalitis), which is suggested by symptoms such as altered mental status, seizures, or focal neurologic deficits” appears twice with two sentences in between.

Mechanism evaluation:

VPedigo planned to add information about the susceptibility of certain populations, which is now discussed in depth, and a video, which is also present. I particularly liked the addition of the video, which I think does a good job of giving an overview of meningitis in an engaging way.

Diagnosis evaluation:

VPedigo planned to a picture of a lumbar puncture and add a paragraph discussing the differential diagnosis for viral meningitis, both of which were done. I think that hyperlinks could be added to each disease entity under in the “further considerations” sentence, but this is a very minor point.

Treatment and Epidemiology evaluation:

A paragraph about possible hospital intervention and more updated epidemiology statistics were both added as planned. Under the epidemiology section, I do wonder why more adults were affected in the viral meningitis outbreak in Romania and Spain when it is also stated in the same paragraph that children tend to be more affected than adults. Perhaps more explanation could be given with regards to that outbreak. Also “While, people aged younger than 15 made up 33.8% of cases” is a fragment.

Overall evaluation

Overall, VPedigo made some excellent contributions to this article. The content that was added is relevant, up-to-date, accessible, and well-organized. I particularly like the media that was added to the article; both the video and picture make the article more engaging. I think that adding the updated information about epidemiology was particularly important and helpful, considering that the incidence of cases each year is increasing (per the article).
