Should be able to 'specify' whether the abbreviated form is enabled

There are a couple of awkward conditionals in the prose, especially because the reader can't be expected to know if [the abbreviated form is] "enabled" (or, indeed, required).

It would probably be better to either:

1. Add a parameter to this template saying whether or not that form is enabled
2. Enable it on all relevant templates and drop the conditional
