Template giving incorrect readings for plagiarism

The template seems to be giving an incorrect reading for the plagiarism parameter. See Template:Did you know nominations/Murder of Michael Nigg. I put y to indicate the article is free of plagerism or close paraphrasing, but it outputs a blue ?. See below too. ~ ONUnicorn(Talk|Contribs)problem solving 14:42, 3 September 2015 (UTC)Reply

Unable to give additional comments next to green checks

When I tried using this template at Template:Did you know nominations/San Nicolò all'Arena, Verona, I wanted to be able to give some additional comments on items that were approved, but this template doesn't seem able to let me do so without adding x's. "Other problems"

Putting a check mark after "Other problems" makes it look like there are other problems. Of course common sense tells me it means that there are actually no other problems, but I think it would be more logical if it actually said "No other problems:  "
