A COVID-19 vaccine card is a document provided to one who has received a COVID-19 vaccine. The card provides basic information, such as the recipient’s name, date of birth, date(s) of shot(s) received, type of shot received, and the location where the shots were given. It can serve as proof of one’s vaccination when this is necessary.
