A Wikipedia article about an organization may not be based primary on, let alone copied straight from, its website. The sole independent source you've cited that isn't a directory-type listing supports only the single fragment "provided blood pressure screenings, presentation on health and wellness to seniours at the Concord Senior Center", which does not make an encyclopedia article. The subject does not appear to meet the notability guidelines for inclusion in Wikipedia. Wikipedia is not for advertising, marketing, or public relations.
