ICD-10 vs ICD-10-CM

I've amended the codes you added at neuroendocrine tumor in this change[1]. The codes added are from ICD-10-CM, which have their own parameter and template. Though the {{ICD10CM}} template has existed since early 2018, it was only recently (June) made explicit within the {{medical resources}} documentation that {{ICD10}} template must not be used to display codes from the national varients of the ICD (and vice-versa, in the case of ICD-10-CM).
