• You did not use the correct unblock template to request a rename. I have corrected this.
• Are you aware of the risks of using your real name, and do you accept them?
• You won't be unblocked if your only intention is to write about yourself. Wikipedia requires all topics to meet its notability criteria to merit inclusion. It is very difficult to be an objective judge of your own notability, and write about yourself from the required neutral point of view.
