@Wmbata can you fix some bugs. 1) The request for page protection requires as username (maybe remove that and SPI). 2) There are too many options to select, Vandalism and Vandalism-only are basically duplicates and custom isn't needed. @Wmbata for the reasons, one option is "Vandalism", could you change the text of that report to "vandalism after final warning"? The options "Username issue" and "suspected sockpuppet" aren't needed.
