My guess is that the three things mentioned are all exclaves, with the first being Madrid Bend, and the second maybe Madha. The controversial structure might be either Ceuta border fence or Melilla border fence, which protect the exclaves of Ceuta and Melilla respectively.
I reached this guess by looking at emirates that might have been ruled by doctors, and found Sharjah, which got me to Madha and the possibility that what was being looked for was an exclave. Exclave describes Madrid Bend as having been formed by the largest earthquake in the continental US.
I don't feel all that confident about what the third exclave should be, however. There seem to be a lot of exclaves, many of which have some sort of barrier to protect them from encroachment by neighboring territories. However the two examples I listed seem to fit in that the walls which separate them from Morocco are, according to their articles, objects of controversy.

- I'll butt in here as I wrote this question. The Melilla border fence is indeed the correct answer - the link between the Melilla exclave and the other two is that they are the three articles in the 'exclave' category that begin with the letter 'M'.
